If using Excel for input, your file must look like the example files available
for download above. Note the column headers must be named EXACTLY the same in
order for this utility to read your data.
If you are having trouble with the Excel file format, save your input file as
an XML file by opening the file in Excel, choosing File -> Save As and
picking 'XML Spreadsheet' as the Save As Type. Submit the resulting .XML file
picking XML from the drop down list in step 2.
Windows XP SP2 Users: If you are having trouble recieving your output, it may
